The idea takes shape
Years of managing aging parents' health across disconnected portals, paper records, and missed hand-offs made the problem impossible to ignore. Development on Titan.Care began early 2024 — built first for ourselves, then realizing every family needed it.
2025
Core platform ships
Medication tracking, appointment management, care circles, and family health profiles go live. Multiple caregivers can now collaborate around one person's care with role-based access.

Epic MyChart integration live
Completed full Epic MyChart integration — connecting Titan.Care directly to real hospital records across 400+ health systems. Epic-approved and live for all users.

Atif built Titan.Care from a place every adult child of aging parents knows too well. Managing his parents' health meant juggling four different hospital portals, handwritten medication lists that were always slightly out of date, and the creeping dread of not knowing what the other siblings knew. One parent on blood thinners, another with a condition that changed specialists every year. A cardiology appointment here, a lab result buried in a portal he could never remember the password to. The breaking point wasn't one dramatic moment — it was the thousand small ones. The 11pm call trying to remember which pharmacy had the current prescription. The ER visit where no one on the care team had the full picture. The realization that the most important information about the people he loved most existed nowhere in one place. He couldn't find the tool he needed, so he built it — and Titan.Care was born.

Hina's story is one millions of caregivers know but rarely talk about. As a working mother managing her own household and the relentless health needs of aging parents — chronic conditions, rotating specialists, non-stop appointments, medication schedules that change without warning — she found herself drowning in a sea of portals, paper forms, and phone calls that never quite connected. Lunch breaks spent chasing referrals. Evenings deciphering discharge summaries. Weekends coordinating between siblings who each had a piece of the picture but nobody had the whole story. Titan.Care was built for her — and for every caregiver carrying that invisible weight.
